Aged care · Support at Home
Coordinated support for comfort, choice and time at home.
The End-of-Life Pathway is a short-term Support at Home pathway for older people with an estimated three months or less to live who wish to remain at home. It is designed to complement—not replace—state and territory palliative care services.

How this pathway may help
Understand the pathway before making a decision.
We can explain the general pathway, listen to what matters to you and help identify a practical next step. Assessment decisions and service approvals are made through the relevant aged care process.
Care shaped around the person
Planning can include the person, family or supporters, their medical team and palliative care services where involved.
Extra short-term support
An approved pathway provides additional in-home aged care support for a defined period, alongside other appropriate care.
Clear communication
The focus is on understanding preferences, coordinating services and helping everyone know the next step.
What happens next
Start with a clear conversation.
We can discuss your circumstances, explain what information may be helpful and connect you with the appropriate next step. We do not decide eligibility or guarantee funding.
