The Restorative Care Pathway is a short-term Support at Home pathway that can provide intensive allied health and/or nursing services. It is built around goals that matter to the person, such as moving more safely, managing daily tasks or returning to valued routines.

How this pathway may help

We can explain the general pathway, listen to what matters to you and help identify a practical next step. Assessment decisions and service approvals are made through the relevant aged care process.

Goal-focused planning

Work with your care team to identify practical goals and the supports that may help you reach them.

A short, intensive period

An approved episode can provide up to 16 weeks of services. The assessment outcome confirms what is available.

Connected clinical support

Services are selected to match assessed needs and may involve allied health and nursing professionals.
